Job Summary:
Responsible for evaluating individual and family needs after receiving a diagnosis and create a custom treatment plan. Provides individual, couples and family therapy to improve mental health and coping skills. Connect families and caregivers to institute and community resources needed to improve quality of life.
Job Duties:
- Evaluates individuals and family's needs after receiving diagnosis and create a custom treatment plan and provide individual, couples and family therapy to improve mental health and coping skills.
- Facilitates and leads various therapeutic and support groups for patients and caregivers to develop appropriate coping mechanisms.
- Develops innovative support and community engaging programs for caregivers and patients to promote mental health and encourage involvement in programs.
- Coordinates with other Providers, agencies and community resources to provide comprehensive care.
- Connects families and caregivers to institute and community resources needed to improve quality of life.
- Performs all other duties as assigned.
Education:
Master's degree in Psychology or related degree is required.
Experience cannot be substituted for degree.
Experience:
Two (2) years of Counseling, Social Work or related experience is required.
Additional Licenses and Certifications:
Licensed Professional Counselor (LPC) is required.
